FOOD ELIGIBILITY GUIDELINES

  • Medicare Cards for everyone in the household (physical card) – passports/visitor paperwork/working visa – each visit you must show for every member in the household
  • Proof of Address – this is not a necessity
  • Source of Income (no need for paper copies, just verbal)
    • type of weekly/bi-weekly income for the household
  • Expenses (no need for paper copies, just verbal):
    • Rent/Mortgage
    • Power/Phone Bill
  • Food services will be available Monday to Thursday from 10:00am to 1:00pm
  • Each client will have an annual review to update information
  • Clients are asked to provide change of information when required, if prior to their annual review
  • Each client will sign the Disclosure Form
