ALL INFORMATION IS HIGHLY CONFIDENTIAL.
FOOD ELIGIBILITY GUIDELINES
- Medicare Cards for everyone in the household (physical card) – passports/visitor paperwork/working visa – each visit you must show for every member in the household
- Proof of Address – this is not a necessity
- Source of Income (no need for paper copies, just verbal)
- type of weekly/bi-weekly income for the household
- Expenses (no need for paper copies, just verbal):
- Rent/Mortgage
- Power/Phone Bill
- Food services will be available Monday to Thursday from 10:00am to 1:00pm
- Each client will have an annual review to update information
- Clients are asked to provide change of information when required, if prior to their annual review
- Each client will sign the Disclosure Form
